Can i attach documentation with a Disabiltyu Update Report from Social Security?

I just received from social security the Disabilty Update Report.I am due for my 3 year review for being on SS Disabilty.
It asks about treatments youve had,meds you take,and Drs youve seen?But it only give 3 lines to write it all and the lines have blocks for each letter?Theres no way of fitting it all?
Do they allow for us to attach any documentation to show whats been done and who ive seen?

Of course they allow for documentation. If there isn't enough room in the "remarks" section then enclose, or attach, a separate piece of paper providing the information. That's what the interviewer would do if they were filling out the form. They only want information regarding the past TWELVE months. It is important to show what is done when you visit your medical sources as it determines whether or not the Disability Determination Service will contact them for medical records. It can also determine what questions the DDS will ask the contacts as well.



If you go to a clinic and see different doctors that is ONE treatment source. Same thing if you go to a hospital.

No need to attach any documents. They will contact the Doctors you have seen to obtain your records.
